Scale layer · CHCII
Connected Health & Community Innovation — proven locally, operational everywhere.
CHCII is the scale layer: proven local models, operationalized regionally and nationally — rural pilots, Puerto Rico and USVI programs, and community-governed AI deployment.
The scale problem
Every good local model runs into the same wall.
The pilot works. The community trusts the team. Then the grant ends, the next region starts from zero, and the lesson is lost. CHCII is designed to end that pattern.
The usual pattern
- Pilot funded — pilot ends — team disbands
- Every new region starts from scratch
- Community trust doesn't travel with the model
- Success is measured in the pilot, not the platform
The CHCII pattern
- Every pilot is a template — governance, workforce, tech
- Next region inherits the operating manual and network
- Community-governance travels through NHIT's coalition
- Success is measured in sustained deployment, not one grant cycle

Rural focus · Georgia
